INNSIDERS VISIT FORM

* Record of visits according to measures required in ORDER 920/2020 of July 28, 2020 of the Ministry of Health of the Community of Madrid, amending the Order 668/2020 of June 19, which establishes preventive measures to address the health crisis caused by the COVID-19 after the extension of the state of alarm established by Royal Decree 555/2020 of June 5.
* In accordance with the provisions of the Organic Law 3/2018, of December 5, Protection of Personal Data and guarantee of digital rights, and by the Regulation (EU) 2016/679 of the European Parliament and the Council, of April 27, 2016, on the protection of individuals with regard to the processing of personal data and the free movement of such data and as stipulated in the ORDER 920/2020, of July 28, 2020 of the Ministry of Health of the Community of Madrid, establishing prevention measures, containment and coordination necessary to address the health crisis caused by the Covid-19, Mi Casa Inn is obliged to collect data from visitors invited by our innsiders and that if required may be made available exclusively to health authorities for the sole purpose of facilitating the tracking and monitoring of contacts in cases positive, probable or possible Covid-19.
The data are collected with the consent of the interested parties, without prejudice to conditioning the right of admission for public health reasons in the event of not being able to rely on it. These data will be kept for a period of twenty-eight calendar days with due guarantees and observing the requirements of the regulations on protection of personal data and will be available exclusively to the health authorities and with the sole purpose of monitoring possible contacts of positive, probable or possible cases of COVID-19.
